TESTIMONIAL RELEASE AGREEMENT

VLXM LLC

Effective Date: May 11, 2026

This Testimonial Release Agreement (this "Agreement") is entered into by and between the individual submitting content ("Participant," "you") and VLXM LLC ("VLXM").

1. Grant of Rights

You grant VLXM a worldwide, perpetual, irrevocable, royalty-free, transferable, sublicensable license to use, reproduce, modify, publish, display, distribute, and create derivative works from your testimonial, including text, images, video, name, likeness, voice, and biographical information, in any media.

2. Consent to Editing

You acknowledge and agree that VLXM may edit your testimonial for

• Clarity

• Length

• Compliance with legal or regulatory requirements

Provided that the overall meaning is not materially altered.

3. Representation of Truthfulness

You represent and warrant that

• Your testimonial reflects your honest experience

• All statements are truthful and not misleading

• You have not made prohibited medical or disease claims

• You are not under obligation that would conflict with this Agreement

4. No Medical Claims

You acknowledge that

• Your testimonial does not constitute medical advice

• Your experience is individual and not guaranteed

• You will not represent your experience as typical or guaranteed

5. No Compensation Disclosure

You must disclose if you received

• Free product

• Payment

• Affiliate compensation

VLXM may disclose this publicly where required.

6. Waiver of Rights

You waive

• Any right to inspect or approve final materials

• Any right to compensation (unless separately agreed)

• Any claims relating to use of the testimonial

7. Release of Liability

You release VLXM from any claims arising from use of the testimonial, including:

• Misinterpretation

• Editing

• Distribution

8. Indemnification

You agree to indemnify VLXM against claims arising from

• False statements

• Misrepresentation

• Violation of law

9. Governing Law

This Agreement is governed by Florida law.

10. Acceptance

Submission of a testimonial constitutes acceptance of this Agreement.
